If you could please print off these forms and have them filled out before your appointment it would be greatly appreciated. I do my absolute best to keep patients from waiting for their appointments. Waiting is not fair to you, me, or the other patients. As you’ll see from the intake forms I like to be incredibly thorough in order to provide the highest quality health care possible. It is very important for you to be as complete and thorough as possible when filling these out. Often times things you think don’t mean anything are actually important clues.
